Coventry EFT Enrollment Due August 1st

Coventry is requiring all providers to enroll in their electronic funds transfer system to continue receiving payments. If you have not completed enrollment on your own or through HCA, you can download the required form below. If you would like HCA to complete the form, please sign it, attach a voided check, and mail it to HCA, attention Pat or Tina. Or, you can complete the form and send it along with a voided check directly to Coventry via fax or email.
