HCA has received a lot of inquiries lately regarding coverage for orthotics from Cigna. Please note that most Cigna plans do not cover custom orthotics when billed with plantar fasciitis (M72.2).

In general, Cigna only covers custom orthotics if prefabricated orthotics will not suffice, and one of the following applies:

Impaired peripheral sensation/circulation
Acquired congenital foot deformity with pain and failed conservative treatment
Neurologic or neuromuscular condition causing malalignment of the foot
The orthotic is part of a leg brace
The orthotic is compensating for a missing part of the foot
